The Preventive Perks Plan is not insurance and is not subject to regulation by the Office of the Health Insurance Commissioner in Rhode Island. Preventive Perks Plan cannot be combined with dental insurance plans. Plan dues are non refundable and do not carry over after the 12 month term expires. No credits or reimbursements will be issued for unused services or benefits. Plan benefits are subject to change and Plan Terms apply.
